Agape Community Health Clinic was established to develop and maintain primary
health care in Beaufort, Hyde, Pamlico, and Martin Counties for all residents
desiring services and especially for individuals with limited access to medical
care.
Agape will provide primary medical care, basic laboratory services, case management, eligibility assistance, and pastoral counseling. We have received special funding that allows us to provide services for a very low co-payment to those who are within poverty guidelines and meet program requirements. At Agape, the medical staff is under
the direction of an Internal Medicine physician and HIV Specialist. We are
also staffed by a Physician's Assistant (PA), a Medical Office Assistant (MOA),
and a Family Nurse Practitioner (FNP). Agape’s business practices are
supervised by an office manager.

The determination of eligibility for the MAP program is based on total household income. An individual must qualify for the MAP Program and cannot have any type of insurance. Proof of income sources and Medicaid/Medicare and Health Choice denials required.
