
Eligibility is offered to Beaufort County residents without insurance who fall within the Access Health of Beaufort County income guidelines.
Referral services to outreach agencies are provided within our program for food, shelter, clothing banks, utilites, and counseling, to name a few. For more information on our partner agencies, see below. Each patient is assigned a case manager and/or health educator to work with him or her individually. The need for preventive health care in place of emergency room crisis care is the focal point of this program.
Access Health of Beaufort County also offers a pharmaceutical program. Patients must be below 200% of the official poverty level and have no pharmacy coverage to be eligible. Seniors with Medicare Part A are eligible. Please contact us for more information.
The staff is comprised of two Nurse/
Care Managers and three Care Managers who will provide one-on-one services.
These skilled individuals work diligently to insure that each patient receives
the care, instruction, and education on disease management and other outreach
services that he or she needs and is eligible for.
The support staff consists of a Pharmaceutical Coordinator who works hard
to secure medicines at reasonable rates and administrative staff who ensure
the success of the program.
